Minor Adaptations and Low Level Equipment
This procedure should be used by:
- Occupational Therapists;
- Occupational Therapy Assistants;
- Anyone trained as a Trusted Assessor (TA).
Minor adaptations are those that are easily installed and do not require structural changes to the home. These can include things like grab rails, stair rails and external rails. Minor works will cost under £1000.
Low-level equipment is a small piece of specialist equipment which can be used to prevent, reduce, delay or that help an individual manage everyday tasks at home, such as preparing food, getting out of bed or visiting the toilet.

A proportionate assessment of need must be completed before making a decision about the appropriateness of low-level equipment and/or a minor adaptation.
When the most appropriate equipment and/or minor adaptation is identified the TCES Connections online ordering system should be used to place the order.

Step 1
Identify equipment needed during a proportionate assessment.
Step 2 (Non-prescriptive items)
Non-prescriptive equipment should be ordered using the TCES System and details recorded on case notes in Mosaic.
TCES will only allow you to order equipment that you have the authority to order.
If further authorisation is required TCES will prompt for this.
Step 2 (Prescriptions)
Where possible it should be arranged for the item to be issued from an accredited pharmacy of the person's choice. The person can then either collect the item themselves or arrange for another person to do so on their behalf.
If the person is unable to collect the item or arrange for it to be collected, you should contact the pharmacy to establish if they are able to deliver.
Only when the pharmacy is unable to deliver should delivery then be arranged as per non-prescriptive items.
Step 3
Follow up with the person to review the effectiveness of the equipment and provide support if necessary.

Be sure to include relevant information, such as where minor adaptations are to be installed, diagrams, and relevant measurements.

Follow up with the person to review the effectiveness of the adaptation and provide support if necessary.
Any routine maintenance is scheduled by the TCES system and carried out automatically by Medequip when it is due.
If equipment provided by Medequip requires repair the person should contact Medequip directly on 020 8709 7050.
Equipment provided via a prescription becomes the property of the person. If it breaks, they should notify First Response. If there is still a need for the equipment another prescription should be issued.
It first needs to be established whether the equipment is of a nature to be recycled. If so, a request for collection will need to be raised via the Medequip system where Medequip will arrange for item to be collected.
If the equipment is not recyclable the person should dispose of it themselves.
If request for equipment is as a result of the person passing away, then a request for collection can be made directly to Medequip on 020 8709 7050.
Once prescription has been redeemed the equipment becomes the property of the person and does not need to be returned. If no longer needed the person should dispose of it themselves.
