Scope of this chapter
The Reablement Team delivers a time-limited holistic physical therapy service to support people to regain their strength and confidence.
The Reablement Team work to the reduce, prevent and delay agenda as set out in the Care Act.
The Reablement Team works with people who are referred via health and social care teams.
