About the Hospital Team

The Hospital Team supports all Haringey Residents admitted to Hospital that are referred for more complex care and discharge planning.

The Hospital Team work to prevent, reduce and delay as well as the assessments and support planning and integration agenda as laid out in the Care Act.

Customers are referred directly from the Single Point of Access when they require complex care management and multidisciplinary working to facilitate a safe transfer of care back to the community.
